Bản 1.0 — Ban hành kèm Quyết định số 0909.CEO/2026/QĐ-MAJOR ngày 9 tháng 9 năm 2026 của Chủ tịch Công ty CP Major Education.

Đây là tài liệu giáo viên dùng nhiều nhất. Đọc cùng Tài liệu 03 (6 bước) và Tài liệu 02 (Lằn ranh đỏ). Bản tiếng Anh ở phần sau.

Cách dùng ma trận

Ma trận không phải bảng "lỗi A = xử lý X". Cột khắc phục là gợi ý đã qua 5 câu hỏi vàng; người lớn vẫn phải đi đủ 6 bước, và học sinh được đề xuất cách sửa trước khi người lớn đưa lựa chọn.

Thứ tự ưu tiên: dạy trước (Ma trận Kỳ vọng) → giải quyết cùng em (Plan B) → khắc phục hậu quả → ghi nhận biện pháp theo Thông tư 19 khi đủ điều kiện. Biện pháp Thông tư 19 là hồ sơ pháp lý, không phải mục tiêu.

Mức độ do người lớn tại chỗ xác định theo tính chất và hậu quả (Điều 12 Thông tư 19); ma trận ghi mức thường gặp, có thể lên hoặc xuống một mức theo hoàn cảnh.

Nhóm C: an toàn trước, báo Leader cơ sở trong ngày, đi theo Tài liệu 07. Không đi tuần tự Mức 1 → 2 → 3.

How to use the matrix

This is not an "offence A = response X" table. The repair column is a suggestion that already passes the 5 Golden Questions; you still walk all 6 steps, and the student proposes a repair before you offer options.

Priority order: teach first (Expectations matrix) → solve with the student (Plan B) → repair the harm → record the Circular 19 measure when the conditions are met. The legal measure is a record, not the goal.

The adult on the spot decides the level by nature and consequence (Circular 19, Art. 12); the matrix shows the usual level, which may move one level up or down with circumstances.

Group C: safety first, report to the Campus Leader the same day, follow Document 07. Do not step through Level 1 → 2 → 3.
